Services Provided by Lake Whitney Home Health: Nursing Care RN and Vocational nurses come into the patient's home to assist the patient and caregiver on disease process, whether it is a new or old diagnosis a skilled nurse can assist you. The patient and the caregiver can count on the skilled nurse to assist with wound care, intravenous Infusion therapy, and monitoring and assisting with medications. Home Health Aides Our Home Health Aides work directly under the supervision of a registered nurse. The Aide will assist the patient with Activities of Daily Living, and provide light house work as assigned by the RN. Social Services Lake Whitney Home Health can provide a Licensed Social Worker to work directly with the patient, family and caregiver. A social worker has the ability to put the patient in touch with community resources, and assist with financial concerns and needs. Physical Therapy Lake Whitney Home Health has physical therapists to go into the patient's home and evaluate the patient for needs of physical therapy. The therapist can teach the patient exercises to continue to use even after services are ended. Medical Supplies & Equipment Medical supplies and durable equipment can be delivered to the patient's home, for the patients to meet all of their needs. Qualifications for Lake Whitney Home Health **You must be an eligible Medicare beneficiary and the home health agency must participate in the Medicare program. **A physician must determine that you need home health and sets up a home health care plan with the home health agency, so that your needs will be met. **You must need a certain type of care: intermittent/part-time skilled nursing and physical therapy. **You do not have to be bed fast or confined to the home with never an absence from the home. You may be absent from your home for infrequent periods and short duration, like a trip to the barber, a walk in the yard, or a visit to your doctor. Leaving home must be a considerable and taxing effort. While leaving home you must have an illness or injury that requires the use of an assistive device such as a walker, cane or wheelchair.
